Medication to be Given During School Hours

Parents may request that medications be administered to their children during school hours.  Requests for medication will be treated as any other confidential school information.

1.                A written physician’s order must accompany each prescribed medication to be dispensed. No prescribed medication is administered by school personnel without specific written instructions from a physician. The physician's order should include: Name of the prescribing physician, phone number of physician, name and dose of medication, purpose of the medication, time or special circumstances under which the medication should be given and length of time for which medication should be given. Orders may be faxed to 570-874-2857.
2.               Parent/guardian must sign a Parental Authorization Form for the administration of medication. A written note giving permission for medication to be administered is also acceptable.
3.               All medication must be brought to school in the original container. Pharmacy containers must be clearly labeled with the child’s name, name of physician, date of the prescription, name and telephone number of pharmacy, name of medication dosage and frequency of administration.
