Health Forms

Dental Exams:

Dental exams are required for children in 1^st and 3^rd grades. These exams can be completed by your private dentist or the school dentist. Private exams may be completed up to one year prior to the beginning of the school year they are required. A link to the Private Dental Exam Form is below. Children who have not provided a private exam will receive a school exam during the second half of the school year.

Physical Exam:

Physical exams are required for children in Kindergarten and 6^th grades. These exams can be completed by your private doctor or the school doctor. Private exams may be completed up to one year prior to the beginning of the school year they are required. A link to the Private Physical Exam Form is below. A form from your doctor is also acceptable. Children who have not provided a private exam will receive a school exam during the second half of the school year.

Health Screenings:

Other health screening completed during the school year include:

          Vision screening- all grades

          Hearing screening- grades K, 1, 2, 3

          Height and weight- all grades  

          Scoliosis screen- grade 6

Parents/Guardians will be notified of abnormal results.
